脳卒中片麻痺患者では運動、感覚、高次機能障害に起因するバランス低下がみられる。感覚入力信号に外部からノイズ波形を付加することで、確率論的に入力信号が感覚閾値を超えて知覚しやすくなることが示唆されている。前庭系に対してこのような感覚入力を変調するノイズ前庭電気刺激を用いることで、脳卒中患者の静的立位バランスにおける視覚代償は改善したが、歩行などの動的バランスに対する影響は明らかではない。このため、ノイズ電気刺激が動的バランスに与える影響を明らかにする目的で研究を実施した。
COVID-19の影響により脳卒中患者を対象とした介入が困難であったため、健常若年者15名(19.9±0.5歳)を対象とした。各被験者の感覚閾値の1％(偽刺激)、70％の電流強度でノイズ前庭電気刺激を適用した。立位で電気刺激適用中に能動的に前後への最大重心移動を行わせ、その際の移動距離を比較した。
結果、1％と比較して70％の電気刺激では前後方向への最大重心移動距離は約12％増大したが、統計学的有意性は認められなかった。この結果から、前庭電気刺激のみでは動的バランスへの改善効果は明らかとならなかった。
今年度の当初予定では前庭および体性感覚へのノイズ電気刺激を併用することでの効果を検証することを計画していたが、機器の不具合により前庭電気刺激単独での検証となった。このため今後は、当初予定の複数感覚への電気刺激を併用することでの動的バランスへの影響を継続検証していきたい。
酸素ダイナミクスに基づく大脳皮質微小循環の評価法の開発と検証

本研究は，大脳皮質の神経活動を捉える指標として酸素化ヘモグロビンを測定し，急激な血圧変化を生じさせた時の変動から循環調節に関与する領域を同定すること，導管動脈と毛細血管との差異を明らかにすることを目的に，ヒトを対象とした基礎的研究および実験動物を対象とした基礎的研究の両面からアプローチしている．
2021年度はヒトを対象とした基礎的研究として，2020年度よりも被検者を増やして実施することに加え，動物実験においてヒトでは検証できない現象の生理学的背景の解明に着手した．12週齢のWister系ラットを対象に，イソフルラン吸入麻酔下のラットの腹部大動脈を5分間駆血し，一気に開放することで急激に血圧を低下させた．この時の大脳皮質の微小循環の変化を，麻酔下，開頭下で共焦点レーザー走査顕微鏡により蛍光試薬（Fluorescein isothiocyanate）により標識した赤血球をレーザー光により検出し，高速度撮影することで，毛細血管レベルで捉えた．
ヒトを対象とした研究では，前年度と同様の結果に加え，駆血解放後の血圧最低値までの時間に比べ，酸素化ヘモグロビンの最低値までの時間が有意に遅いことが明らかとなった．この時間には領域間に差はないことを確認した．動物実験においては，血圧低下モデルの作成に時間がかかった．血圧の低下によって大脳皮質レベルの赤血球移動速度は著しく低下することがわかった．一方毛細血管径の変化は，移動速度の変化に比べ小さくかった．
同種造血細胞移植の再発率低下を目指したリハビリテーション法の確立

昨年度に引き続き、同種造血細胞移植後の腫瘍免疫動態を評価を行った。昨年度はHLA半合致移植を中心に解析を行ったが、本年度は同種造血細胞移植を行った造血器腫瘍患者全例を対象として、マルチカラーフローサイトメトリー法を併用し、同種造血細胞移植後30日前後の免疫動態について評価を行った。
その結果、移植後30日時点でのCD8陽性ナイーブT細胞の比率が低く、T-EMRA細胞比率が多い症例で重症急性GVHD発症症例が有意に多いことが判明した。この傾向はドナー種類（末梢血・骨髄・臍帯血）によらず同様であった。このことは、重症急性GVHDを発症する症例は発症前から細胞障害活性を持つT細胞比率が高いことを示している。今回の結果は、aGVHDの発症を予測し、より適切なタイミングでの治療介入を行うことへ向けた重要な所見であると考えられた。
また、本年度は新たに内皮活性化およびストレス指数（EASIX）が予後に与える影響についても解析を行った。適切なリハビリテーションにより、血管内皮機能や血小板活性化の改善が認められるとの報告があることから、EASIXとリハビリテーション、筋肉量との関連性を調べることが重要と考えられる。特に移植前のEASIXのみならず、移植後のEASIXが予後に与える影響について研究を進めており、移植後1年時点でのEASIX(EASIX-1year)がその後の非再発死亡に対する有意な予後予測因子であることを発見した。本研究成果は今後、国際学会での発表も視野に研究を進めているところである。
移植成績には様々な要因があるため、適切な予後予測因子を同定することがリハビリテーション介入において重要である。そのため、同種造血細胞移植全般の予後予測についても並行して研究を進めている。

We examined the effects of exercise and nutritional intervention in patients with hematological malignancies. The 15 subjects were devided into 7 patients with the exercise intervention only and 8 patiens with the exercise + nutrition intervention. Muscle strength (grip strength, knee extension strength), endurance, muscle mass, simple nutritional status, malaise, and QOL were measured at the admission and 3 months after admission. Exercise consisted of endurance and muscle strengthening training for 20 minutes a day, 6 days a week. Nutrition intervention used two supplements a day. In the exercise + nutrition intervention group, knee extension strength was maintained 3 months after admission compared to the admission. However, there were no significant differences between the two groups in terms of grip strength, endurance, muscle mass, MNA, and QOL.
